Healthier Kids Foundation collaborated with the County of Santa Clara to launch AppointmentFirst in November 2020. This program supports the County’s efforts to ensure foster children receive required medical and dental exams while placed in foster care.
Our Case Managers assist caregivers by scheduling and managing the children’s medical and dental appointments. We may assist caregivers in finding a medical or dental home for foster children if they do not have one established. Case Managers collaborate with the children’s assigned Social Worker to overcome obstacles that may prevent them from completing all required exams successfully.
